Athletic Clearance

Athletic clearances are open for the 2025-26 school year
through Home Campus. 

Carefully read the entire section to ensure a successful clearance process: 

1. Complete a physical exam AFTER June 15 using the GUSD Physical Exam Form.
2. Complete the Home Campus online athlete clearance. Instructions are available for download in English and Spanish.
3. Upload the completed GUSD Physical Exam Form & Proof of Insurance to Home Campus.**
4. The GHS Athletic Department will review and approve completed online clearances in a timely, but not instantaneous, manner.
5. Once cleared, you will be sent an email with instructions to pick up your emergency card from the Student Activities Office.  

* Note: All GHS football players must have a valid ImPACT baseline test on file. This test must be renewed every two years. After completing the Home Campus athlete clearance process, contact athletic trainer Carolina Garcia (carolina.garcia@gilroyunified.org) to schedule the online test.
